County Health Unit, 201
E Hempstead, Nashville.
This will be a walk in
clinic with curb service for
handicapped individuals or
those needing assistance.
Please pick up forms at the
health unit and complete
prior to coming for faster
service. Please bring your
insurance or Medicaid/
Medicare cards.
